Ultrasound of joints: what does it show, when is it prescribed, how is it done?

Ultrasound diagnostics have been used in medicine for quite a long time, and the emergence of new high-tech digital devices equipped with high-frequency sensors has made it possible to use this method in rheumatology.

Nowadays, ultrasound of joints is one of the advanced methods for diagnosing diseases whose symptoms are characterized by joint pain. This technology is absolutely safe, affordable, and, importantly, provides the patient with complete information about the problem.

When is it necessary to do an ultrasound of the joints?

An ultrasound of the joints of the lower or upper extremities must be performed if the patient has one or more indicators:

  • injuries, both recently acquired and the presence of injuries in the past;
  • autoimmune diseases;
  • obesity;
  • acute or chronic inflammatory diseases;
  • systemic diseases;
  • pathologies of the endocrine system;
  • osteochondropathy (with X-ray confirmation or if it is suspected);
  • pain in the area of the compounds;
  • restrictions on motor capabilities in the joints;
  • swelling of the joints or increase in their size;
  • screening studies of the hip joint of newborns at risk;
  • suspicion of hip dysplasia (in newborns);
  • suspected subluxation or dislocation of the hip joint in infants or children under 1 year of age;
  • observations during therapy for dislocation, dysplasia, subluxation of the hip joints in children under one year of age;
  • muscle pain;
  • palpable mass formation in soft tissues;
  • observations during the treatment of inflammatory diseases of the joints and tendons (tenosynovitis, arthritis, tendinitis);
  • suspicion of a cyst or rupture of a Becker cyst;
  • observations for bursitis.

Why do an ultrasound if an x-ray is done?

Traditional radiographic examination allows one to evaluate only the bone structures of the joint, but does not visualize the soft tissues of the joint and cartilage. The source of pain in rheumatic joint diseases is inflammatory processes in the soft tissues, so it is necessary to carry out both radiography and ultrasound.

Ultrasound or radiography of joints?

Each diagnostic method has its own disadvantages and advantages. If there is a choice, then first of all the shortcomings and how many there are are taken into account. X-rays of joints are quite significantly inferior to ultrasound examination and have the following disadvantages:

  • Low information content of the research procedure in children under 3 months of age. This is due to the incomplete formation of bones, which at this age consist mainly of cartilage and so on until 6 months.
  • X-ray does not make it possible to evaluate cartilage and connective tissue structures.
  • Such a diagnostic procedure is not carried out in the “here and now” mode, and its repetition is also limited, which cannot be said about ultrasound.
  • Unjustified exposure of the body.
  • The possibility of obtaining inaccurate data due to insufficient fixation (for example, the child may not be able to stand it and move, thereby violating the information content of the study).

Ultrasound examination has a number of advantages over radiographic examination:

  • Full guarantee of painlessness and safety of this method.
  • The method is highly informative from the very first days of a person’s life.
  • The possibility of conducting multiple studies in order to monitor the dynamics of the development of the disease or the effectiveness of treatment procedures.
  • Among the advantages of ultrasound, one of the most important is its mobility. After all, an ultrasound machine allows you to conduct research both at home and in a separate ward of the clinic, and not just in a specially equipped room.
  • This method allows you to view the joints themselves - bone, cartilage and connective tissue, soft structures.
  • Using ultrasound, namely a special color Doppler mapping mode, you can study the state of blood flow in the joints. Which in turn expands the diagnostic capabilities of the method.

What does an ultrasound of joints show?

This technology makes it possible to examine soft tissues that are inaccessible with X-ray examination. Using ultrasound of joints, you can assess the condition of muscles, cartilage, tendons, and joint capsules. In addition, you can identify various tumors, determine cartilage degeneration, see the presence of ruptures, hematomas and other abnormalities.

Reviews about the joint ultrasound procedure in our clinic

Our family is very grateful to the doctors of your clinic. My father has had arthritis for a long time, but the stage was quite difficult to determine. In February, I took my father for an ultrasound at Onmed, and it was here that we finally received a good consultation. Thanks to experienced specialists, the degree of destruction in the joints was determined and treatment was prescribed. And the pain goes away, and movements return every day. Thank you very much! Sincerely, Igor.

Possibilities of ultrasound in rheumatological diseases

Rheumatic diseases are characterized by the presence of an inflammatory process in the joints. Ultrasound shows very clearly the inflammatory fluid accumulating in the joint cavity, as well as in the bursae surrounding the joint. During the study, the severity of the inflammatory process can be assessed by the volume of visible fluid. If there is a lot of fluid, then there is a lot of inflammation in the joint and vice versa. Ultrasound visualizes not only the quantity, but also the structure of the fluid - by the degree of its heterogeneity, one can understand whether the inflammation has been exacerbating for a long time or recently, because over time the fluid becomes thicker and more viscous.

Based on the totality of ultrasound signs, the doctor decides the volume of anti-inflammatory therapy: is it necessary to inject the joint, is it possible to evacuate the inflammatory fluid, which drug is more appropriate to prescribe to the patient, how many injection sessions should be performed.

Indications

Of course, it is worth doing an ultrasound of the joints only after a prescription from the attending physician, and when the following symptoms are observed:

  • characteristic crunching and clicking sounds;
  • pain at the joint;
  • formation of tumor-like formations;
  • limited mobility;
  • swelling;
  • injuries, especially the lumbar region, fractures and dislocations.

Using ultrasound, doctors monitor the condition of joints with osteoporosis, synovial cysts, erosive defects, synovitis and tendonitis, rheumatism, arthritis and other pathologies. Without ultrasound today it is impossible to imagine the process of diagnosis and treatment of all diseases of the musculoskeletal system.

Many people ask the question: why do an ultrasound of the hip joints for a child - an infant who has just turned one month old? The study is carried out to identify congenital pathologies (dysplasia), deviations from the norm, and developmental delays. With early diagnosis, there is a high probability of eliminating the defect so that there are no consequences or reminders left. There is no need to be afraid of the procedure, because it is safe, painless and does not cause any harm to the baby.

In our clinic in the Nevsky district of St. Petersburg, ultrasounds are performed for both children and adults by experienced specialists, inexpensively. The patient receives a document with the data obtained during the study. With it, a person can freely contact their doctor to prescribe therapy.
